To "unlock" Primals, you must clear a lvl 70 Greater Rift solo while beating the timer. Primals have a "hidden" requirement before they can begin generating in your game. Ancient items will not drop on difficulties lower than Torment 1. Items that drop naturally (from monsters, rift guardians, caches, etc) start with a 1% chance of being Ancient at Torment 1, reach 10% at Torment 6, and odds continue to rise with difficulty. Note: Set Items gained from the Cube via Convert Set Item cannot be Ancient.
Items created by Crafting, Gambling, or obtained via Kanai's Cube (such as Reforge Legendary or Upgrade Rare) have a flat 10% chance of being Ancient, and is not affected by difficulty level of the game. However, the odds of getting an Ancient vary based on the method used. Any method used to farm a Legendary or Set Piece item can be used to get Ancient versions as well. There is currently no known way to "force" an item into being Ancient, and there is no way to farm them specifically.
